post-short case

Dear Blog,

Oh snap!

Bodoh gile!

Blank like hell!

Stuttered like an a**!

I apologize to both Dr. Loo and Dr. Azira for having to observe my horrible, horrible short case.

I really blew it. I hope I didn't fail =(((

I got Respiratory System patient:

  • trachea deviated to the right (ipsilateral deviation)
  • reduced chest expansion on the right
  • dullness and reduced breath sound & vocal resonance on the right

differential dx: pulmonary fibrosis????

and RHEUMATOLOGY patient:

  • swellings at the MCP joint
  • symmetrical joint involvement
  • Z deformity, swan-neck deformity
  • ulnar deviation
  • no psoriatic nail changes (eg: nail pitting)
  • loss of functional ability of hands
  • no psoriatic plaques and no rheumatoid nodules

differential dx: Rheumatoid Arthritis (RA)????


weyhh...apa ni? I forgot the proper steps on how to do the hand examination for RA. I was shocked. It stated there on the white piece of paper, with bold letters "EXAMINE THE PATIENT'S HANDS".

After seeing that trigger, bye-bye. Brain shutdown. Ka-boom!

Crap. Please lah, I don't wanna re-sit this exam again. Don't wanna fail! =( The nerve-wrecking moments are hard to tolerate with.
